I think it's because you are not in Japan at the moment. Most non-Japanese people only know sushi, unfortunately. So, basically, it's about selling food.
I think the most unhealthy food are those not prepared properly. You'll end up with salmonella or dead (in the case of puffer fish and extremely bad food poisoning)
References :
dude most japanese restaurants don't serve sushi. i will admit that a lot do but not most. i have been to many restaurants in japan that don't serve sushi. they serve food like a variety soups, yakisoba,TEMPURA OMG THAT IS SO GOOD, udon noodles, many noodle dishes, gyoza japanese style, japanese style pizza. yes sushi can be a very healthy meal if you know what your eating and now what good sushi looks like
References :
living in japan for 3 years

Most Japanese restaurants serve a lot of sushi because that's what their customers going there want. Some Japanese restaurants that are larger and more upscale also have a varied menu of kitchen cooked foods like terriyaki,or udon. Some with grill rooms make a whole evening of entertaining while cooking your meal.Probably the least healthy type of food is the fried foods, like tempura, which is breaded and deep fried.
References :
Although sushi bars serve mainly sushi, even there you can get soup, seaweed salad, dumplings, that sort of thing. A lot of american japanese restaurants also have hibachi (which is like japanese barbeque or a japanese steakhouse).
Hibachi is sort of healthy (although it depends on the kind of oil they use, the kind of meat you order (they have beef, chicken, shrimp, tofu) and the kind of rice you choose. Clear japanese soups, as well as miso soup are also healthy.
